You should
know that Summit Internal Medicine is striving to become a Patient Centered
Medical Home by the National Committee on Quality Assurance (NCQA). This means that we work as a team to provide
your healthcare. You, as the patient,
are an integral facet of our partnership as we work to deliver you the best
care possible.
Our practice follows evidence-based
guidelines in patient care. Examples
include guidelines established by the American Diabetic Association and the
Joint Committee on Prevention, Detection, Evaluation and Treatment of
Hypertension. With these guidelines in
place, the care team can give you access to self-management support.
As a patient centered medical home,
Summit Internal Medicine will help coordinate appointments, testing and
procedures for you. We will communicate
with specialists we refer you to and ensure that your transition of care is
secured.
